Mr. Kolasa’s policy is to have an initial “no cost” consultation with most clients who request it. Either call Mr. Kolasa right now to schedule an appointment, or review and complete the online “Request an Appointment” form and Mr. Kolasa will contact you directly. You may also wish to print out and complete the attached Estate Planning Questionnaire and present it to Mr. Kolasa at the first meeting. This is not a mandatory form, but is provided for clients needing a written questionnaire to organize his or her thoughts.
